You need -Wundef or -Wall to check for undefined preprocessor symbols.
$ sparse -Wundef test.c
test.c:1:5: warning: undefined preprocessor identifier
'SYMBOL_NOT_DEFINED'
test.c:4:2: warning: "not defined"
