I believe I heard it verbally either from Peter Salus or perhaps I read the paper long ago also and I'd have sworn I had it in my files but I drew a blank on my short search. I thought it was Kernighan's paper but I could well be wrong. I'm pretty sure it did show some "sample" code. It certainly described the whole process.
