Another issue: `--' gets replaced with `—'. The debian cogito
package mentions this. Seems it can be fixed by overwriting the default
replacement rule using the following in asciidoc.conf:
[replacements]
(^|[^-])--($|[^-])=\1--\2
Maybe it can be done smarter.
