First step to move gdth and thus one of the two remaining drivers off
the scsi_request interface:

Introduce a gdth_execute abstraction to send a firmware command to a
host.  This gets rid of all the knowledge of scsi_requests or scsi_cmnd
structures everywhere in the drivers (and thus also the big ifdef mess).

As a next step gdth_execute can be switched to a different underlying
mechanism.
