I know it's a pot shot, but I have
"alias snd-card-1 snd-bt-sco"
in /etc/modules.conf.

Alsa's configuration still mystifies me, I have the same versions of just 
about everything you do, very similar configuration (albeit a gentoo system) 
and my headset (Sony Ericsson HBH-IV835) is working fine.

Might be worth giving the modules.conf line a shot anyway.
