Where's the best place to send/propose/discuss these?
What's the preferred format?

The best place to send patches is this mailing list. I think they have to be a unified diff, i.e. diff -u.

Hello Gary,
Just send the patches to this list as unified diffs, preferably inlined to the email to ease reviewing. If the patch is large consider splitting it into several patches and emails.

I'd also recommend using the '-p' option, which shows the name of the
function that each change is in.
