You should be able to feed pv-grub with a menu.lst that fetches the
kernel and initrd from the cdrom, yes; i.e. put in the config file

kernel=/usr/lib/xen/boot/pv-grub-x86_64.gz
ramdisk=/tmp/menu.lst
disk=...
vif=...

where /tmp/menu.lst contains for instance

root=(hd1)
kernel /vmlinuz
initrd /initrd.img
boot
