we are about trying to reproduce this problem and will keep you up-to-date on 
our findings.
Can you also give us more detailed information, i.e. does this happen
with hap only or also with shadow paging ?

The changeset 
http://xenbits.xensource.com/staging/xen-unstable.hg/rev/e7afe98afd43
touches generic p2m code. The EPT code definitely needs an update in
respect to get EPT-on-EPT with nested virtualization.
Until further analysis is done, I can not say, there is no bug in the p2m
changes and I can say as well that you might need Eddies EPT update patch
as this may contain the fix you need.
