Hi,
Since this file became a hot topic [*], I took previous cleanups
from last year and continue to clean it.
v2:
- reorder fw_cfg (Laszlo), remove forward declarations
- move PixelFormat to "ui/qemu-pixman.h" (Paolo)
- reword comment (Thomas)
