FYI,
nothing formal but we usually keep whole change log here in form
Changelog:
   vN+1:
       - ...
       - ...
        entries are basically a copy from per patch chagelogs
   vN:
       - ...
   ...
   v2:
       - ...
    
this way reader can see what has changed without need to
look for all previous versions.
