To eliminate dispatching NMI/IRQ through the IDT, add
kvm_vmx_reinject_nmi_irq(), which calls external_interrupt()
for IRQ reinjection.

Lastly replace calling a NMI/IRQ handler in an IDT descriptor
with calling kvm_vmx_reinject_nmi_irq().
