The quilt patch titled
     Subject: mm: move dummy_vm_ops out of a header
has been removed from the -mm tree.  Its filename was
     mm-move-dummy_vm_ops-out-of-a-header.patch

This patch was dropped because it was merged into the mm-stable branch
of git://git.kernel.org/pub/scm/linux/kernel/git/akpm/mm

Otherwise the kernel ends up with multiple copies:
$ nm vmlinux | grep dummy_vm_ops

ffffffff81e3c780 d dummy_vm_ops.7

While here prefix it with vma_.
