The quilt patch titled
     Subject: mm/mmu_gather: remove unused __tlb_remove_page()
has been removed from the -mm tree.  Its filename was
     mm-mmu_gather-remove-unused-__tlb_remove_page.patch

This patch was dropped because it was merged into the mm-stable branch
of git://git.kernel.org/pub/scm/linux/kernel/git/akpm/mm

Nobody is using __tlb_remove_page() now, clean it up.

And also remove the code comment above tlb_remove_page() because it's not
meaningful any more.
