This is a retransmission of a patch sent on 2004/10/23
titled: "[PATCH] scsi_debug  v 1.75 for lk 2.6.9-bk4".
It still applies to 2.6.10-rc2 and I can't see that it
has been picked up in the linux-scsi bk archive.

Changelog:
     - fix highmem data transfers
          - fix kunmap_atomic() argument
          - disable clustering
          - sets resid
     - allow every_nth < 0 for error continuously once
       count is reached
     - minor sense buffer handling cleanup
