Subject: [PATCH] update additional sense codes and some opcode names

Following on from my post titled: "additional sense codes
need update" see the attachment against lk 2.6.17 .

ChangeLog:
  - update additional sense codes table to agree with
    SPC-4 revision 5a  (14 June 2006)
  - adjust some of the opcode names
