The recent change from mmio physical addresses to offsets break sparc 
fault handling because the offset is now passed to do_unassigned_access 
which puts the offset into the fault address register.

This change also makes debugging unassigned addresses more difficult 
because of the missing physical address.

I don't see an easy way to fix this problem other than reverting back to 
physical addresses.
