More testing:  Bare virtual box with just piix+vga.  Then place a virtio 
disk into different slots using addr=.  3,4,5 work fine.  6 gives me:

ACPI: Unable to derive IRQ for device 0000:00:06.0
ACPI: PCI Interrupt 0000:00:06.0[A]: no GSI - using IRQ 9
  vda: <hangs here>

Looks like a irq routing issue ...
