[PATCH] opensm/libvendor//osm_vendor_ibumad_sa.c: uninitialized fields

Taking care of couple of uninitialized fields.
Discovered with 'osmtest -f a' - bad things
start to happen when a packet is dropped, and
osmtest should really get a timeout.
