If you enable CONFIG_IPIPE_DEBUG_INTERNAL, do you see the following message:

[  292.460803] kernel BUG at arch/arm/kernel/ipipe.c:445!

(the line number may be a bit different as I am testing Xenomai on Linux
3.14).
