Almost. In your .gitconfig change
url = git@github.com:dslutz/qemu.git
to
url = https://github.com/dslutz/qemu.git
pushurl = git@github.com:dslutz/qemu.git
It will also be faster for everyday use.

Yes, this looks good.

Instead of pushing a branch, do
git tag -s -f for-upstream
<type tag message>
git push --tags name-of-github-remote
and git request-pull should just work.
