     blk_mq_realloc_hw_ctxs

If 'tag_set' expansion failed, 'set->nr_hw_queues' will fallback 
previous  value. So If we don't
call '__blk_mq_free_map_and_rqs' to release the newly allocated hardware 
context will lead to
memory leak.
