So I reviewed this on top of my gdbstub/next series so needed to make
some tweaks as I didn't have the riscv misa_mxl patches from above
because there still seems to be some debate about them.

If we can get a series that doesn't depend on them (or the RiscV guys to
agree with the changes) with the review comments addressed I think I'm
ready to pull at least 1-15 into my tree for the next pull request.

I want to spend a little more time looking over the plugin API itself
and experiment with some alternatives. But I don't think that need hold
up getting the gdbfeature stuff squared away now.
