I would hope the compiler saw through the duplicate indirect accesses
but maybe:

    uintptr_t *ptb, ntb;

and

    ntb = *ptb;
    n1 = ntb & 3;
    tb1 = (TranslationBlock *)(ntb & ~3);

would be clearer?
