It's also (partially) documented here [1], though I don't see it explicitly
stated anywhere that new versions must use "-v2", "-v3" etc., and it also
states that for new versions "you *can* add it to the same thread"[2].
We need to tighten up the documentation to make it clear that subsequent
versions of a patch "must" use a version number and that they "must" be
sent in-reply-to the original thread.
