v1 to v2:
 - Change KERNEL_OUTPUT to KERNEL_OUTPUT_DIR and update comments
 - Update related doc files
 - Replace all KERNEL_ALT_IMAGETYPEs with KERNEL_IMAGETYPES
 - Link built vmlinuz to boot directory for reference
