[POWERPC KVM] Implement KVM for PowerPC 440

Avi, these patches have now been acked by Paul Mackerras, so I think they're
ready to be committed.

Changes from v2:
- Move some code into booke_guest.c
- Actually include booke_host.c this time

Changes from v1:
- Updates for latest KVM upstream (build fixes)
- Split some code out into booke_host.c

Changes from v0:
- Move FPRs into struct kvm_fpu
- Use u64 for register size in kvm_regs
- Create Documentation/powerpc/kvm_440.txt
- Comment describing incompatibility with PPC_EARLY_DEBUG
- Replace hardcoded mask in kvmppc_44x_tlb_shadow_attrib() and add comment
- Comment to explain why we special-case DCRN_CPR0_*
- Don't redefine DCRN_CPR0_*
- Slightly reword description for patch 2
