Get blktap2 running on pvops.
Syncing blktap2/pvops with linux-2.6-xen. This queue merges a bunch of
blktap2 related changes from linux-2.6.18 into the pvops tree.

Different from the filenames listed, results are far from
pristine. Where differences matter, comments went into the header.
I didn't test in detail which one are stricly required on pvops. But
least the tlb flushing updates apparently were a good idea.
These were all done all against xen/master @ 60e0545e
