dmaengine: idxd: return -ETIMEDOUT from drains and resets and leak what the device may still write

This commit updates idxd_wq_drain(), idxd_wq_reset(), and
idxd_device_drain_pasid() to return -ETIMEDOUT when a command times out or is
refused after a timeout, and changes their callers to retain memory and PASID
bindings that the device might still access. It also marks workqueues as stuck
after a failed drain in idxd_dma_synchronize() so descriptor slots are not
reused while the device may still complete in-flight work.

[Severity: Critical]
When drained is non-zero in idxd_file_dev_release(), jumping to put_wq skips
idxd_xa_pasid_remove(ctx) in addition to iommu_sva_unbind_device(ctx->sva),
leaving ctx reachable in wq->upasid_xa after the file is closed and the
process exits.

Neither idxd_cdev_open() nor iommu_sva_bind_device() takes an mm_users
reference on ctx->mm (iommu_sva_bind_device() only takes mmgrab() on
mm_count). Once the process exits and exit_mmap() tears down the VMA tree and
page tables of ctx->mm, a subsequent event log completion record fault for
ctx->pasid will run idxd_evl_fault_work() -> idxd_copy_cr():

drivers/dma/idxd/cdev.c:idxd_copy_cr() {
	...
	ctx = xa_load(&wq->upasid_xa, pasid);
	if (!ctx) {
		dev_warn(dev, "No user context\n");
		goto out;
	}

	mm = ctx->mm;
	kthread_use_mm(mm);
	left = copy_to_user((void __user *)addr + status_size, cr + status_size,
			    len - status_size);
	...
}

Can this access a destroyed mm_struct (or write into reused virtual addresses
if the process only closed the file descriptor), and also cause xa_insert()
to fail with -EBUSY if the same process reopens the cdev after a late command
completion? Since keeping the IOMMU SVA binding already prevents the PASID
from being reallocated, should idxd_file_dev_release() still remove ctx from
wq->upasid_xa before leaking ctx and ctx->sva?

[Severity: High]
When a drain or reset command in idxd_cmd_exec() is ended by a concurrent
device reset or halt recovery via idxd_cmd_reset_state(), idxd_cmd_exec()
sets *status = IDXD_CMDSTS_HW_ERR:

drivers/dma/idxd/device.c:idxd_cmd_exec() {
	...
	if (idxd->cmd_done != &done) {
		spin_unlock(&idxd->cmd_lock);
		dev_err(&idxd->pdev->dev,
			"device command %#x was ended by a reset\n",
			cmd_code);
		if (status)
			*status = IDXD_CMDSTS_HW_ERR;
		return;
	}
	...
}

However, in idxd_reset_done() (after a PCI FLR), idxd_cmd_reset_state(idxd)
is called after the hardware reset has already completed, so GENSTS.state is
already IDXD_DEVICE_STATE_DISABLED (or IDXD_DEVICE_STATE_ENABLED once
re-enabled), not IDXD_DEVICE_STATE_HALT. Likewise, in idxd_device_reinit(),
idxd_cmd_reset_state(idxd) is immediately followed by idxd_device_reset(idxd)
and idxd_device_enable(idxd), which clear GENSTS.state out of HALT
concurrently with the woken caller.

When the woken caller then runs idxd_cmd_drain_result(idxd, status),
idxd_device_is_halted(idxd) will return false and idxd_cmd_drain_result()
will return -ETIMEDOUT even though the reset already killed all in-flight
descriptors in the device. Won't this cause idxd_file_dev_release() to leak
ctx and its PASID binding, idxd_drv_disable_wq() to leak descriptor memory
and leave wq->state enabled, and idxd_dma_synchronize() to put back
quarantined descriptors and set wq->stuck = true after idxd_wq_halt_release()
has already run?

[Severity: Critical]
If idxd_wq_drain(wq) fails with -ETIMEDOUT in idxd_drv_disable_wq(), the code
still unconditionally calls idxd_wq_free_irq(wq) and idxd_wq_reset(wq).

First, idxd_wq_free_irq(wq) calls idxd_flush_pending_descs(ie, false) with
quarantine set to false:

drivers/dma/idxd/device.c:idxd_wq_free_irq() {
	...
	free_irq(ie->vector, ie);
	idxd_flush_pending_descs(ie, false);
	...
}

In idxd_flush_pending_descs(), when quarantine is false, any pending
descriptors are completed with free_desc = true, which calls
dma_descriptor_unmap(tx) and idxd_free_desc(wq, desc). If the drain timed out
and the device is still active, won't this unmap the client's DMA buffers
while the device may still be reading or writing them?

Second, unlike idxd_wq_disable() which skips IDXD_CMD_RESET_WQ when
idxd_wq_drain() fails, idxd_drv_disable_wq() still calls idxd_wq_reset(wq).
If idxd_wq_drain() failed because an earlier command had timed out
(IDXD_FLAG_CMD_TIMEDOUT), and that command completes late in
idxd_misc_thread() while idxd_wq_free_irq() is running, idxd_wq_reset(wq) can
execute IDXD_CMD_RESET_WQ on the undrained queue and succeed. That resets the
WQ without draining translations and runs idxd_wq_disable_cleanup(wq)
(setting wq->state = IDXD_WQ_DISABLED), while rc |= idxd_wq_reset(wq) remains
non-zero so idxd_wq_free_resources(wq) is skipped. Can this leave the WQ
reconfigurable via sysfs while its old descriptor resources remain allocated?

[Severity: High]
idxd_dma_synchronize() splices wq->quarantine onto its local stack list, runs
idxd_wq_drain(wq), and then updates wq->quarantine and wq->stuck without
holding wq->wq_lock, whereas idxd_wq_halt_release() and idxd_drv_disable_wq()
serialize using wq->wq_lock.

If a device halt happens right after idxd_cmd_drain_result() checks
idxd_device_is_halted(idxd), idxd_wq_halt_release(wq) can run under
wq->wq_lock while the quarantined descriptors are still on
idxd_dma_synchronize()'s stack:

drivers/dma/idxd/device.c:idxd_wq_halt_release() {
	...
	guard(mutex)(&wq->wq_lock);
	if (wq->state != IDXD_WQ_ENABLED || wq->type != IDXD_WQT_KERNEL)
		return;
	idxd_wq_quarantine_take(wq, &dead_descs);
	idxd_wq_quarantine_free(wq, &dead_descs);
	WRITE_ONCE(wq->stuck, false);
}

idxd_wq_halt_release() will see an empty wq->quarantine and set
wq->stuck = false, and then idxd_dma_synchronize() will splice the
descriptors back into wq->quarantine and set wq->stuck = true, leaking those
descriptors and wedging the queue after the halt.

Similarly, if a client calls dmaengine_synchronize() directly while
idxd_dmaengine_drv_remove() -> idxd_drv_disable_wq() ->
idxd_wq_free_resources() runs under wq->wq_lock, can idxd_wq_free_resources()
free wq->descs and wq->sbq while idxd_dma_synchronize() still holds
descriptors in its local quarantined list, leading to a use-after-free when
idxd_wq_quarantine_free() frees those descriptors back to wq->sbq?
