The whole handling of placement.lpfn looks rather dubious... if I'm
reading it right, we're usually applying the VRAM size as a limit for
GTT as well? I guess your change doesn't make it any worse, but we
should probably clean this up at some point.

Couldn't it still work in the CPU domain? Though I guess a BO that can
never be moved into GTT or VRAM isn't very useful...
However, if the size exceeds GTT, I think this needs to compare against
visible_vram_size instead of active_vram_size, or it may not be possible
to map the BO?
