The main change is just to maintain the glossary manually instead of by
the sort_glossary.pl script; includes a subsequent patch to demonstrate
the sort of cleanup that allows.

(Erm, I think I forgot to say they're in the "maint" branch in that
repo.)

It don't see these in the latest maint or master, but I don't think the
following discussion turned up any objections.  Let me know if there's
something more you need from me.
