Unify remote request (TLB_FLUSH and MMU_RELOAD) with kvm_vcpu_kick mechanism.
The new wait_on_bit based scheme also allows finer optimization of
different request types, and makes future enhancements easier (such as
request to a single vcpu, or subset of vcpus).
kernel compilation is 2.5 - 3% faster with shadow paging.
