Relocate rdtsc{,_ordered}() from <asm/msr.h> to <asm/tsc.h>, and
subsequently remove the inclusion of <asm/msr.h> in <asm/tsc.h>.
Consequently, <asm/msr.h> must be included in several source files
that previously did not require it.
