A few fixes/changes to round off this merge window. This pull request
contains:

- Small series making some functional tweaks to blk-iocost (Tejun)

- Elevator switch locking fix (Ming)

- Kill redundant call in blk-wbt (Yufen)

- Fix flush timeout handling (Yufen)
