Add clk_pll_mmc_ops for enable/disable, prepare, rate control, and status
operations on MMC PLL clocks.
Also add clk_pll_mmc_phase_ops to support phase get/set operations.

Changes in v2:
- Added missing Co-developed-by tag.
- Replaced eval_ssc_div_n() function with a macro PLL_MMC_SSC_DIV_N_VAL.
- Removed clk_pll_mmc_disable_unused() and clk_pll_mmc_unprepare_unused()
callbacks to rely on CCF default behavior and avoid unnecessary function calls.
- Fixed clk_pll_mmc_determine_rate() to properly write the calculated rate into
req->rate.
