If I have a device that has a 'modalias' file in sysfs, should it
show up as ENV{MODALIAS} in the uvent? Or should I check for
both the environment variable and the sysfs file?

So, if it shows up as 'modalias', but not as ENV{MODALIAS}... udev
bug? kernel bug for the subsystem?

Run:
udevmonitor --env
If it isn't there, it should be added to the kernel in the subsystem
uevent callback.
