Is there any real documentation about the formats of the usb.*map files? 
Specifically I'd like to know what all those fields mean and how to 
write a new record.

Same as the /lib/modules/$(uname -r)/modules.usbmap output.
Docs for the content is found in curent 2.5 <linux/usb.h>, and how
its mapped to the usbmap format is in the modutils source code.
