submitted bug #535191 to fix Phillips USB camera hotplug

As an FYI ... I submitted bug #535191:

on the SourceForge 'Feature Requests' (I did not see a public bug tracker, so
I used the 'feature request' forum instead).

This is a revisit of a earlier message that I sent to the mailing list.
We have been using this patch successfully for a number of weeks and
recommend this patch be added to the main code for the next release.

I added a few bits of it to the tree, the big changes I'll let David
comment on before adding them.

I commented separately as followup to the original email.
Some updates should be in CVS soonish (the server's not
talking to me just now) -- one bugfix touching on one issue
mentioned here, and comments.
