Subject: PATCH: usb-storage: fix hotplugging, and bugs

Attached is a patch to usb-storage which fixes the hotplugging to use the
new tables. JE, please review and forward to Linus and Alan Cox.
This patch also fixes the bug where we would ignore the alternate interface
probing and only examine altint 0.
The patch seems much larger than it is -- the patch moves some data from
usb.c to unusual_devs.h, which makes it seem much larger.
