Here's a first pass at a lvm2create_initrd script. I basically modified
the LVM1 version of the script, using the initrd image created by
Christophe Saout as a model, and especially borrowing his linuxrc
script.

It has some caveats that are detailed in the README. Like the prior
script, it's released under the GPLv2.

It also supports a 'lvmrescue' mode that runs a shell at the end of the
linuxrc script. Very helpful for fixing problems with your root volume
group or root filesystem.

Patches are gratefully accepted!
