Full disclosure: when testing this with fluster, I had a kernel 
segfault.

After some debugging I noticed that this bcm2835_codec_flush_buffers() 
call (which waits until the above v4l2_m2m_buf_done() callbacks are 
completed; is not actually correctly waiting for it, specifically: the 
frame_cmplt completion is actually completed per vbuf in the callbacks. 
This is possibly the culprit for the issue. the stop_streaming comments 
mention that vb2_wait_for_all_buffers() may be used; but I like the 
timeout here and was thinking of doing a reinit_completion, until the 
counter is 0, much like the wave5 driver is doing that.
