Do the initial settings of the fields in common code. This will allow
us to push more processing into common code later and improve readability.

V1->V2:
 - slab_common.c Remove #ifdef and use proper targeting for the goto.
 - Remove superfluouis free of s->name.
 - Remove the refcount move to slab_common (seems to be buggy)
