  I have the same problem. It is really  a clock problem. Try to disable
the CONFIG_OMAP_RESET_CLOCKS options:

System Type  ---> TI OMAP Implementations  ---> [ ] Reset unused
clocks during boot

  I don't know if this error is in the USB subsystem or if it is a bug
in the code
that resets the clocks at boot time.
  I would like to know if it worked for you...
