change_queue_depth callback API changed
The change_queue_depth callback changed where there is now an additional
parameter called reason, with SCSI_QDEPTH_DEFAULT, SCSI_QDEPTH_QFULL,
and SCSI_QDEPTH_RAMP_UP codes.
