This removes all the east users of the command serial number.  After
this we still have dpt_i2o, which uses the serial number for finding
commands in it's completion handler, and the eata and u14-34f drivers
which use it for their own elevator algorithms.

The isd200 sub-driver increments the command serial number despite not
using it at all in it's routine for sending internal scsi commands.
Remove the increment to prepare for removing the serial_number field.

There's no real point ... it's a useless number unrelated to anything
else in the system (there's nothing you can really use it for in
debugging).

Plus once all of these patches are through, we'll eliminate the serial
number field from the host and command.
