I found a copy of 6.01.00-fo, so now /proc/scsi/qla2300/3 reports: QLogic PCI to Fibre Channel Host Adapter for ISP23xx: Firmware version: 3.01.13, Driver version 6.01.00-fo and the three disk volumes are found and are available. I guess the driver downloads the firmware, as this is different from the firmware reported with driver version 7.00.03-fo So it looks like the disk array doesn't like the newer qla2300 driver/firmware ...
