The APs rotate key index 1 and 2 for broadcast keys as you said, but
I've never seen it use anything but key index 3 for unicast keys. Key
index 0 is apparently never used. 

I understand that rekeying broadcast keys while all STA's exchange
broadcast traffic has a synchronization problem. But for unicast traffic
there is no way to lose packets during rekeying since at any given time
only one packet can be in the air. Am I missing something?
