The PHB state PNV_EEH_STATE_REMOVED maintained in pnv_phb isn't
so useful any more and it's duplicated to EEH_PE_ISOLATED. The
patch replaces PNV_EEH_STATE_REMOVED with EEH_PE_ISOLATED.
