Not nice.

fs/binfmt_misc.c:bm_register_write() is doing d_instantiate()
under a write_lock(), and the selinux d_instantiate() hook is
performing sleeping functions.

Now, we can just move the d_instantiate() call to outside the lock:

proc_pid_lookup() calls d_add->d_instantiate under task->proc_lock, so
inode_doinit_with_dentry() is called under spinlock on this path as well.

Manfred, is there any particular reason why proc_pid_lookup()'s d_add is
inside the lock?

AFAICS, we can move d_add() right before taking the spinlock.  It's there
to protect the ->proc_dentry assignment.

*However*, I would like to point out that we are holding ->i_sem on the
procfs root at that point, so any blocking code in d_instantiate() would
better be careful to avoid deadlocks if it wants to play with procfs itself -
we are not in a locking-neutral situation here, spinlock or not.

In which case we don't need to take the lock at all.  Two instances.

What protects against concurrent execution of proc_pid_lookup() and
proc_task_lookup()?  I think nothing, because one is at /proc/42 and the
other is at /proc/41/42; the parent dir inodes are different.  hmm.

Yes, we do.  At least we used to - the other side of that code assumes
that holding the spinlock is enough to keep ->proc_dentry unchanged.
And no, I hadn't done the analysis of changes that had come with the
"task" ugliness.

Ugs.
/proc/1 and /proc/1/task/1 are two different dentrys.
proc_task_lookup happily overwrites task->proc_dentry. Which means the 
task patch broke tsk->proc_dentry.
I think the cure is simple: proc_task_lookup should not write 
proc_dentry, only proc_pid_lookup should do that.
tsk->proc_dentry is only used by proc_pid_flush: If a task exits, all 
entries below /proc/<pid> are stale, and a shrink_dcache_parent on the 
/proc/<pid> dentry recovers the memory.
There is a race between in proc_pid_lookup between checking that the 
task is still running and setting tsk->proc_dentry, but AFAICS the race 
is not critical: In the worst case, the stale dentries remain around. 
They are never returned to user space, d_revalidate prevents that.

The lifetime of dentries is quite long, thus I'd prefer if the race 
between proc_pid_lookup and sys_exit is closed.

This shouldn't be a problem for SELinux, because the /proc/pid inodes
are initialized by proc_pid_make_inode via the security_task_to_inode
hook (=> selinux_task_to_inode), so inode_doinit_with_dentry will bail
immediately on the first test of isec->initialized prior to any blocking
calls.

I asked Al Viro about this issue back when the proc locking change was
introduced (circa 2.5.70), and he seemed to agree that the SELinux code
is safe in this case.  He was concerned about the change in behavior for
d_instantiate, but d_instantiate seems to be the more general location
to perform inode security initialization for the majority of filesystem
types; hooking in iget() would only handle a subset of filesystems.
