Logging a message when the driver is compiled with debugged would be useful.
But for the bigger picture, does this give you any ideas as to how I can try
to reproduce this more reliably? Parallel iwconfig eth1 in multiple terminals
doesn't seem to be giving the code any grief, even though it hits all the
ioctls. events/0 is doing a scan each time, can that be forced from userspace?

Although my understanding of locking has been shown to be deficient, it looks to me as if the
deadlock involves a WX call while a scan is in progress. The following patch uses
mutex_trylock rather than mutex_lock so that WX calls are rejected if the lock is already held.
Please try this patch with wpa_supplicant and NetworkManager both running. I have tested it with
wpa_supplicant alone.

I don't think this is the right thing to do. This cannot be causing the
deadlock as for a deadlock you need (at least) two locks. Now, since any
calls from userspace to d80211 acquire the mutex first, they should be
fine. They also both acquire the rtnl lock first.
The deadlock must be somewhere deeper. Can we run this with lockdep
enabled? Might give us a hint before we dig out all the used locks
here...

If you don't want to go that far, just run a bunch of "iwlist eth1 scan"
and "iwconfig eth1" over and over and you'll effectively simulate the
behavior of NM in this situation (although doing it a lot quicker than
NM likely).
Setting up a script to just run those two in parallel continuously is
probably a good test of any WE locking in a driver :)

I have had lockdep enabled both times, and it didn't whinge about anything.
I'm wondering if that's because events/0 is doing delayed work on behalf of
bcm43xx (it was in both traces as well as NM and wpa_supplicant).
