I have tried it with kernels from debian between 2.6.18-5-686 -
2.6.25. Be it vanilla versions/sources from kernel.org or debian built
kernels. I have the same issue.

The problem is random TCP connection failure for ipv4.

I have seen a severe issue of this problem in my transparent proxy
setup. This problem also occurs at times when it is not redirecting
traffic, rather accepting it
directly. Increasing the number of file descriptors didn't help. I
rebuilt the kernel with 65K file descriptors, still the problem
persists.

Even if it is one client behind squid or this box, this happens. It
will at random, not accept connections. squid doesn't report any in
the log. netstat doesn't show any
in its output as well.

I have changed NICs, I have changed between squid versions from 2.x to
3.x. Nothing has helped.

Following are some settings to mention;

ulimit -n is set to 8000
net.core.somaxconn = 1024
net.core.netdev_max_backlog = 3000
net.ipv4.tcp_max_syn_backlog = 3000

squid is built with 8000 file descriptors.

This is a 60 client LAN behind this box, using it as an internet
gateway/firewall for the following traffic;

1) vpn
2) email
3) cvs
4) browsing (http/https)
5) some ftp

Is the proxy server accepting connections after the issue has occurred?
Or do you need to re-boot? Or do you have to wait for a number of
seconds or what?

Yes, after a while (it can be a few seconds, minutes, or 1+ hour) the
proxy server starts accepting connections again. I don't need to
reboot the box when this happens.

This happens randomely to anyone behind it. Then, after a while, it
starts working again by itself.

Well, there are too many possibilities for failures... the Linux
distribution (RHEL 3 workstation) was built for 2.4.21 (around year
2003) and you try 2.6.x

Maybe you should stick to one of the later 2.4.x kernels or even better
upgrade the Linux distribution.

I didn't have problems with rhel ws3 or with the kernel it came with.
I removed it in favour of debian a long time back, when debian/etch r0
was released.

I have been running debian etch r2 (4.0 r2) ever since, which comes
with kernel (2.6.18-5-686). As you might notice from the mentioned
kernel versions, I tried upgrading to kernels from lenny/debian and
sid/debian hoping to find a solution. I tried from 2.3.13 - 2.6.25
from both debian packages and vanilla sources (as they are called),
but the problem persists. It started and is continuing on debian.

I have 2 ISPs hooked into this box. One is CIR, the other is ADSL. The
ADSL runs over pppoe. I don't know if the 2.4.x kernels support pppoe.
The 2.6.x kernels do. I need to run 2 ISPs on this box seemlessly.

The last rhel version I used was using kernel 2.6.9. I'm going to grab
that and see if that helps.

I need pppoe, iptables, vpn, scsi with raid support, smp, and a around
8000 file descriptors to run stable on this box.

So, you have the same issue with all tested kernels
RHEL 3 works, Debian fails

This smells configuration failure, you better compare your working
configuration with the others.

When I was using RHEL WS3, which comes with kernel 2.4.21-27.EL
(sorry, the 2.6.9 comes with rhel 4), it worked ok. I haven't tried
new kernels on rhel ws3 though.

I tried all the mentioned kernel versions on debian. All show the same problem.

Note sure if its config or what, but this problem exists.
